Enclomiphene is a selective estrogen receptor modulator (SERM) that stimulates your body’s natural testosterone production. Yes. Enclomiphene maintains or even improves fertility because it stimulates natural testosterone and sperm production through LH and FSH. Unlike TRT, which can suppress sperm production, enclomiphene works with your body’s systems. Studies show it can improve sperm parameters while raising testosterone levels.

Enclomiphene can produce gains comparable to TRT. Some men go from 200 ng/dL to 1500 ng/dL. Results vary based on age, baseline levels, testicular function, and dosing. Response is typically apparent within 2-4 weeks and can be optimized through dosing adjustments.

Enclomiphene is prescribed off-label for testosterone optimization. While not FDA-approved specifically for male hypogonadism, it has extensive safety and efficacy data. The manufacturer withdrew their FDA application for business reasons, not safety concerns. Off-label prescribing is common for effective medications.

If TRT raised your testosterone but caused intolerable side effects or fertility concerns, enclomiphene may be a good alternative. However, if your testicles can’t produce testosterone adequately (primary hypogonadism), enclomiphene likely won’t work well since it stimulates production rather than replacing the hormone. Enclomiphene works best for secondary hypogonadism (when the signal from the brain to the testicles is weak) but not for primary testicular failure. Lab testing helps determine which type you have.
